The Dean of Students Office receives and responds promptly to reports involving Academic Integrity, Basic Needs Referrals, Bias Incidents, CARE Referrals, General Student Misconduct, and Natural Disaster Support. When appropriate, the office collaborates with other campus departments to ensure a coordinated response.
The Office of Equal Opportunity and Title IX handles reports related to Discrimination, Discriminatory Harassment, Sexual Harassment, and Retaliation. This office also works in partnership with other campus departments when needed to address reports effectively.
Academic Integrity: This report form is for reporting an academic integrity violation of the Student Conduct Code by a Santa Clara University student. Faculty are directed to use this form when reporting an academic integrity violation in their course. Anyone else who has information about an academic integrity violation is invited to use the form to file a report. Basic Needs Support: This form is designated to support students who are experiencing food insecurity—the lack of reliable access to sufficient quantities of affordable, nutritious food. Through this program, students are able to receive access to the Bronco Pantry and donated funds through the Dean of Students Office. Find more information about the Basic Needs Program. CARE Referral: This form is for referring a student who may be struggling with academic, personal, or emotional difficulties or who may be exhibiting concerning behavior. Students are encouraged to use this form as a self-referral if unsure where to get help. Referrals may be filed, including anonymously, by students (including those that wish to self-report), faculty, and staff via the CARE Referral Form.
General Student Misconduct Reporting Form: This report form is for reporting other types of student misconduct that are not consistent with the expectations in the Student Conduct Code. More information about the Student Conduct Code and the University's Statement of Community Values is available in the Student Handbook which can be accessed online under the Dean of Students Office website. Natural Disaster Support: This form is designated to support students who have fallen victim to a natural disaster such as a wildfire, hurricane, tornado, etc. and may be in need of additional support. Students can submit the form on behalf of themselves or a peer, and faculty and staff can submit the form on behalf of a student. Upon receipt, DSO will reach out to the student directly to offer support and assistance.
Discrimination, Discriminatory Harassment, Sexual Harassment, and Retaliation: This form is designed to provide students, employees, faculty, vendors, visitors, or others with an online method to report specific information related to an alleged incident(s) of discrimination, harassment, sexual misconduct, or retaliation.
