22604. Long Term Disability
Statement
Santa Clara University employees are covered by a voluntary long term disability insurance plan provided by the University.
Definition
When a benefits eligible employee, who, as a result of an injury or sickness, becomes unable to perform the material duties of their regular occupation and when in the care of an attending physician, is deemed by the University’s long-term disability insurance provider to be Totally Disabled for 360 consecutive days.
Eligibility
All Santa Clara University employees working at least 20 hours a week in a regular position, and faculty members working under a formal phased retirement agreement approved by the Office of the Provost and Executive Vice President, are covered under the University’s long-term disability insurance plan.
Benefits
Eligible employees will receive 66 2/3% of covered earnings, up to a maximum benefit of $10,000 per month. Long-term disability benefits will be reduced by any dollar amount received as worker’s compensation, social security, disability benefits from another insurance company, retirement plan, settlement agreements, and any wages earned. Specific individual amounts of long-term disability benefits to which an eligible employee may be entitled will be calculated in accordance with the long-term disability insurance plan document.
Duration
Benefits will not extend beyond the longer of: Social Security Normal Retirement Age or Duration of Benefits below:

Certification
A claim form must be filed no later than 90 days after the 360-day elimination period of the occurrence of the disability. A completed Attending Physician Statement signed by the individual’s doctor and documentation from other benefits sources applied for must accompany the claim form.
Responsibility
It is the responsibility of the employee to contact the supervisor and the Department of Human Resources to notify them of the disability. In order to receive long-term disability benefits, the employee must ensure that the proper certification has been filed with the University’s long-term disability plan administrator.
